As a Licensed Massage Therapist (LMT), I follow the Code Of Ethics dictated by the State of Florida.
The confidentiality of all client information both written and verbal will be upheld unless disclosure is required by law or court order. All Platforms used for my business are Health Insurance Portability and Accountability Act (HIPAA) compliant.
A HIPAA covered entity may disclose PHI to law enforcement without the individual's signed HIPAA authorization in certain incidents, including: To report PHI to a law enforcement official reasonably able to prevent or lessen a serious and imminent threat to the health or safety of an individual or the public.
An established atmosphere of trust and respect will be upheld during every interaction, I ask that all clients return the same respect.
It’s important to me that clients feel comfortable and safe physically, emotionally, psychologically and spiritually. Please use your voice if something I say or do makes you uncomfortable.
New client paperwork will be emailed to you to be completed before your first appointment. This will include a liability release, medical history form, cancellation policy acknowledgement form, and COVID-19 waiver. If you are unable to complete the paperwork prior to your scheduled appointment, please inform me and time will be added to the scheduled appointment at no extra cost for you to complete it on location.
During your appointment, we'll review your medical history and any problem areas you may have (pain, injury, range of motion, etc.). We'll also talk through any questions you may have and what to expect. Your first appointment will last approximately 15-minutes longer than the service time so paperwork can be filled out.
A phone consultation might be required before your first session. If so, I will contact you at least one day before your scheduled appointment to talk about your goals, understand your needs and to pre-screen for contraindications.
When I arrive for your appointment, I will call from 727-282-5343 to let you know I have arrived.
Cancellations
24-hour advance notice is required when canceling an appointment. If you are unable to notify me of a cancellation with at least 24 hours notice, you will be charged 50% of the amount of your scheduled appointment. Appointments canceled within 3 hours of the scheduled appointment time will be charged 100% of the amount of your scheduled appointment.
Illness-related Cancellations
If you feel ill, please do not hesitate to reschedule your appointment. For the time being, fees incurred for cancellations made with less than 24 hours notice will be waived. No-call, no-show penalties still apply. Please don't hesitate to inform me if you're not feeling well so we can reschedule your appointment. Additionally, please allow 2-weeks before scheduling your appointment after you no longer experience symptoms of an illness.
This is a healing environment and there is a Zero-Tolerance Policy for inappropriate behavior and comments.
In the event that a client displays any behavior that includes, but is not limited to, clients that are intoxicated, violent, ill, display inappropriate behavior, use sexual innuendos, hate speech, exhibiting harassments, and those requesting services outside my scope of practice. The massage session will end and the client will be required to pay the full amount of the booked massage session.
As an LMT, I have the right to end and refuse service to any client. I reserve the right to call the police and file a police report at my discretion.
Draping
Professional draping will be used during the session – only the area being worked on will be uncovered. In general, massage is given while you are unclothed, however you may choose to dress to your level of comfort.
For health and safety, EPA sanctioned disinfectants are used in cleaning the equipment between each client visit.
Masks
At this time a mask is optional. I am vaccinated. I have no problem wearing a mask if it makes my clients feel safer. Please feel free to discuss this with me if it is a concern for you. Please reschedule your appointment if you are feeling unwell.
It is the responsibility of the client to disclose medical history on an intake form to ensure that there are no contraindications (a condition or circumstance that indicates that massage is not safe).
Clients are asked to include past surgeries, injuries, trauma, and anything else necessary to note. Remember massage therapy affects your entire body, so something you might think is small might dictate what type of treatments are safe for you to receive.
Clients are responsible to update their LMT when their Medical history has changed.
With some medical conditions, massage may not be advised or may need to be altered. If massage therapy could be potentially harmful to the client, I have the right to decline performing the massage.
Clients may be asked to provide a written release from a physician, chiropractor, physical therapist, etc, that massage therapy is safe for you to receive.
The massage you are going to receive is for the following purposes: stress reduction, pain reduction, relief from muscle tension, and/or increasing circulation. I do not, at any time, diagnose or prescribe medical treatment or pharmaceuticals, nor are spinal manipulations part of massage therapy.
I ask that clients please reschedule if they are feeling ill, have a sore throat, cough, fever, or other transmissible contagion. Massage therapy is contraindicated while sick. It is the safest option for you, myself and other clients. Likewise, please understand if I need to reschedule due to illness.
